Our mobile lab comes to you. Our Advanced Blood Panel is our most extensive testing panel which is the key to ultimate health, wellbeing, and longevity. This testing checks 13 sex hormones, 8 thyroid markers, Glucose and HbA1c, Vitamin D, Folate, B12, Iron, Magnesium, Hematology panel, Inflammation markers, Lipids, and a Comprehensive Metabolic Panel that checks all organ functions.

Vision, Dental & Hearing Exam

Original Medicare (Medicare Part A and Part B) does not cover routine dental or vision care. As part of your Sage Health membership, we will provide you with these annual exams in the comfort of your own home.

Our call center is available 24/7 and you will be able to speak to a nurse that can help answer any clinical questions and triage an acute clinical need.

24/7 Telehealth Consults

For those times that you prefer to “see” a clinician via video instead of at home, our virtual care solutions platform are meant to supplement our care delivery, but never replace in-person comprehensive primary care.

Personal Tablet with Sage Health App

We will provide you with your own personal tablet so you can conduct virtual visits, access your patient portal, request prescription refills and eventually book an on-demand appointment via the Sage App.

Remote Patient Monitoring Equipment for CHF, COPD, Diabetes and other chronic conditions

Managing chronic conditions such as COPD, Diabetes and CHF just became easier. Our RPM devices are Bluetooth-enabled devices that provide remote patient care that monitors your chronic conditions and helps prevent an acute exacerbation.
