Home-Based Primary Care (HBPC) offers more time for patients and our providers to assess the home environment and may defer or eliminate the need for care in institutionalized settings. HBPC also provides quality, consumer-centered care for people underserved within the current healthcare system where the consumer must travel to the provider. Besides filling a critical access gap, we believe that providing primary care in the home shifts the entire healthcare experience for the better. It has proven to significantly reduce medical emergencies, fragmented care, missed appointments, and the poor control of chronic conditions.
