Omar has 20 years of specialty provider-facing healthcare experience in commercial operations in Home Infusion, Specialty Pharmacy, PBM, and Health Insurance in Fortune 25 organizations at Accredo Health, Medco Health, Express Scripts and Cigna. Working in those matrixed organizations over 20 years, Omar has collaborated with operations, pharmacy, nursing, payer relations and sales operations and personally saw the negative impact of fragmented care on health outcomes, and the financial and personal challenges of patients with chronic conditions and their caregivers. He currently lives in Marietta, Georgia with his wife and is the father of 3 boys and is an avid saltwater fisherman.
Carla M. Perissinotto, M.D., M.H.S., is a Professor in the Division of Geriatrics, Department of Medicine at the University of California, San Francisco (UCSF) and is board certified in internal medicine, geriatrics, and palliative medicine. She served as the associate chief for geriatrics clinical programs at UCSF from 2017 to 2021. In this role she oversaw and developed new clinical programs serving older adults across care settings. For over a decade she has worked in home-based primary care. More recently, she has begun working in hospice, working to rethink how we care for patients with terminal dementia. Dr. Perissinotto has gained national and international recognition for her research on the effects of loneliness on the health of older adults. Her research and advocacy has focused on integration of loneliness assessments in health care, and evaluation and implementation of community-based programs focused on ameliorating loneliness and isolation in adults.
Shawna Ramey, CPC, CPMA, has extensive national experience establishing and improving home-based practices. She has a passion for the frail, elderly patients in this country. Her mission is to ensure they receive the care they need in the place where they are most comfortable, their home. She has dedicated the past decade to home-based practices and has helped countless providers live their passion by working with them to build sustainable practices. Shawna has a keen eye for creating relationships between vendors and practices showing them how they can work together to enhance the mission of both parties. Additionally, in her role as a coder and auditor, she loves to educate providers and their operations team around documentation and coding opportunities. Always an advocate, Shawna serves on the regulatory workgroup for the American Academy of Home Care Medicine (AAHCM).
